Alot of the incident is not due to careless, sometime due to whatever reason... server roll back, level drop... items that you picked inbetween the period disappeared... How to ensure that there is any roll back or other possibility of error??

Best way is to relog to the game... It's not the most convenient way, it not the fastest way, BUT IT'S THE SAFEST WAY!!
